Activities Award Report Forms

The following forms are to be used as cover sheets for your Best of Quarter and Best of Year activity reports. You may describe your activity on the forms, or you may attach reports to the forms. Note that the forms are no longer color coded and do not have to be printed on colored paper. Be sure the Grand Knight and your District Deputy sign each form, that the correct reporting period is checked, and that the reports are submitted before the deadlines.
